You can print and complete the registration form before your first appointment, then bring it with you. You can also complete it when you arrive. Practice registration and hospital preadmission are separate steps.

These sheets explain recovery after a procedure. You are welcome to read them before or after treatment and save or print a copy. Choose the sheet that matches your procedure. Your discharge paperwork and personalised medication plan take priority over these general guides.

Use only the hospital named in your procedure booking. Your consulting location may be different from your hospital. Completing a preadmission form does not make or confirm a procedure booking.
Open Epworth’s admission information and patient portal. Follow the hospital’s instructions for your booked campus.
Open Mulgrave Private Hospital’s online admission form guidance and continue to its admission portal.
Use the personalised SMS invitation arranged through your doctor’s office. The hospital asks you to complete the portal at least three working days before admission. Read Jessie McPherson’s admission instructions. If your invitation has not arrived, please contact our rooms.
